Cholesterol numbers are usually discussed as though they were a snapshot: a figure from your last blood test, either fine or not fine. That framing hides the thing that actually matters. Atherosclerosis is cumulative. Arteries do not respond to the level you have today; they respond to the total quantity of LDL they have been exposed to across your whole life.1 The unit that matters is closer to an area than a number.

Why the shape of the curve matters

Move the second slider in the tool above and watch what happens. Lowering LDL at sixty barely changes the total, because four decades of exposure have already accumulated and cannot be undone. Lowering the same amount at twenty changes the total enormously. The intervention is identical. Only its position in time is different.

This is the single most useful idea in cardiovascular prevention, and it is the one that public discussion most consistently misses. The benefit of a lower number is roughly proportional to how long you hold it.

What the evidence actually shows

This is not a metaphor invented for a website. Genetic studies point the same way. People born with variants that give them naturally low LDL for their entire lives have far lower rates of coronary disease than the size of their LDL difference alone would predict from drug trials.23 The reason is duration. A drug trial lowers LDL for five years. A gene lowers it for fifty.

That observation is what makes durable, long-acting treatments so interesting, and why researchers have pursued one-time gene edits that lower LDL permanently rather than daily pills taken for decades. It is also why the same tools carry a heavier burden of proof, because a permanent intervention has to be right the first time.

What this does not tell you

The model above is deliberately simple, and it would be dishonest not to say what it leaves out. Cardiovascular risk is not LDL alone. Blood pressure, smoking, diabetes, inflammation, family history, and a dozen other factors all matter, and some of them matter a great deal. Plaque already formed does not disappear when the number comes down, though lowering LDL does slow and can partly stabilise it Approved. Nobody should read a number off this chart and conclude anything about their own health. What the chart is good for is the shape of the idea: that time is a multiplier, and that early matters more than intuition suggests.
